Changes to registration during Covid-19 Pandemic

Due to the current COVID-19 pandemic, patients are asked to register without coming into the practice. To register you will need to complete our online registration form.

 

New Patient Registration

Joining the practice is very simple. Just fill out our online registration form and upload Photo ID, such as your passport or driving license and proof of your address (a Council Tax or utility bill would be ideal).

If you have a preference for a male or female doctor please make this known to the receptionist when you register.

You may be invited to a new patient appointment, particularly if you are on a lot of medication or have complex medical needs. Normally the GP will want to check and review your medication before we start to issue prescriptions.

 

Practice Boundary

Our Practice Area is defined as follows:

  • In town we take in the area to the East & North of a line formed by Rosebarn Lane, Mount Pleasant, Polsloe Road, Fore Street, East Wonford Hill and Quarry Lane.
  • In the rural area we extend out as far as Stoke Cannon, Killerton, Clyst St Lawrence, Whimple, Strete Raleigh, Aylesbeare and Clyst st Mary.

If you submit a registration form and are not living within the practice catchment area, your registration will not be processed.

 

Temporary Registration 

If you are temporarily in our practice catchment area, for example, on holiday or staying with friends, and you need primary care medical assistance, please complete a Temporary Registration Form and submit it to the surgery.
